  • Provide holistic and comprehensive case management services to Impact KCK or KCPS Empowerment clients, including intake assessment (Arizona Self-Sufficiency Matrix), benefits assessment, goal setting, long-term case plan development, weekly case plan development, progress monitoring, individual money management education, tenant housing education, employment education, advocacy, and referrals, and reporting and compliance.
  • Learn and implement case management best practices.
  • Provide intensive strengths-based case management services to a minimum caseload of thirty-five families.
  • Provide emergency assistance to walk-in clients, including resources (food, clothing, hygiene products, etc.) and referrals from community partners.
  • Update client files daily to record continuity of care and maintain complete confidentiality of all client records.
  • Oversee and teach program classes as assigned.
  • Complete data entry into online systems (CCL, MAACLINK, SIEMER , etc.) and remain compliant with grants and data tracking goals.
  • Maintain criteria and reporting requirements on grants specific to housing and utilities.
  • Participate in supervision meetings with the Program Director as required.

Coordination Responsibilities:As part of the collective impact model, the incumbent should expect to coordinate with coalition partners in developing plans and supporting client families.

BSW (Bachelors in Social Work) preferred, but not required.

Our Scheduling Precertification Coordinators serve as a liaison between the patient, clinic teams, and third parties in executing doctor orders, including communicating scheduling information to patients, providing patients with an estimate of services, and initiating prior authorization for patient procedures.

Kennel Attendant

Position:

The Kennel Attendant position is a key role for our kennel that provides daily care for our boarded animals and assist with kennel maintenance. The position involves cleaning, bathing, grooming, exercising and feeding our boarded animals. The Attendant is also responsible for cleaning cages, administering medication, and monitoring the behavior of our boarded dogs. The role interacts with the clinical team, clients and supports our customer focused environment.

Job Responsibilities:

Daily care for boarded animals

Bathing, grooming, exercising, feeding, administering medication, and monitoring behavior

Assist with kennel maintenance and cleaning

Reporting changes in animal behavior, health or concerns to Vet or Hospital Manager

Maintaining animal records, such as weight, physical condition, treatments received, medications given, and food intake and other potential data as needed

Other duties as assigned by Doctor/Hospital Manager/Kennel Manager

Washing, drying, and folding laundry

Must be able to lift 50lbs or more, stoop, bend and stand on feet for long periods.

Travel may be required

Skills / Qualifications:

High School Diploma or relevant education and work experience

Previous experience with animals preferred

Customer service attitude

Organizational skills

Team oriented, enjoys working as part of team

Solid communication skills
